APPOINTMENTS AND CANCELLATIONS
The standard meeting time for the initial visit is 45–60 minutes and follow-up visits are 15–30 minutes.
Payment is due within 24 hours of your appointment. You may lose your appointment if payment is not received within 24 hours of your scheduled time.
Cancellations and re-scheduled visits will be subject to a full charge if not received at least 48 hours in advance. This is necessary because a time commitment is made to you and is held exclusively for you. If you are late for an appointment, you may lose some of the allotted time for that appointment.
Your card may automatically be billed prior to your appointment if payment has not been received.
TELEPHONE ACCESSIBILITY
If you need to contact Pedrotel LLC DBA Thunder Performance Testosterone between visits, please call our main number or send us a message through the website or patient portal. We are often not immediately available; however, we will attempt to return your call or message within 24 hours.
Face-to-face video visits are highly preferable to phone visits. However, in the event that you are out of town, sick, or need additional support, phone sessions may be available.
If a true emergency situation arises, please call 911 or go to your nearest emergency room.
ELECTRONIC COMMUNICATION / TELEMEDICINE
We cannot ensure the confidentiality of any form of communication through electronic media, including but not limited to text messages, telephone communication, the internet, facsimile machines, and e-mail.
Telemedicine is broadly defined as the use of information technology to deliver medical services and information between two parties in different locations. These electronic means of communication are considered telemedicine.
Utilizing telemedicine services through Pedrotel LLC DBA Thunder Performance Testosterone is voluntary, and you understand:
- You have the right to withhold or withdraw consent for telemedicine services at any time. If this occurs, you understand that we may no longer be able to provide care if services are primarily telemedicine-based.
- We will protect your protected health information in the same fashion as a traditional practice; however, data breaches can occur, and we cannot guarantee information is 100% secure.
- We will not use your protected health information for research without your consent.
- There are potential benefits and risks of telemedicine. Benefits include improved access to care, reduced travel, and increased efficiency. Risks include limitations in performing full physical examinations, which may impact diagnostic accuracy.
MINORS
We require parental or legal guardian consent for all visits conducted via telemedicine. A parent or guardian must be present for part of the visit to confirm consent.
Parents may be legally entitled to some information about a minor’s treatment. We will discuss with both the patient and guardian what information is appropriate to share and what may remain confidential.
TERMINATION OF CARE
We may terminate treatment with you at any time after discussing the reasons for doing so. If treatment is terminated, we will provide referrals to qualified providers to continue your care.
If you repeatedly miss appointments, fail to obtain required lab work, or are non-compliant with treatment plans, and no arrangements have been made in advance, we must consider the professional relationship discontinued for legal and ethical reasons.
REFUNDS, AUTO-BILLING, MEMBERSHIP CANCELLATION, AND PAYMENT DISPUTES POLICY
All payments made to Pedrotel LLC DBA Thunder Performance Testosterone are non-refundable. This includes, but is not limited to, consultation fees, membership fees, treatment packages, laboratory services, telemedicine visits, and medication costs once ordered, dispensed, or services rendered.
By enrolling in any membership or recurring service, you authorize Pedrotel LLC DBA Thunder Performance Testosterone to securely store your payment method on file and to automatically bill that method for membership dues, services, appointments, medications, or outstanding balances.
Membership Billing Schedule
• The initial payment will be charged at the time of enrollment or service initiation.
• After the initial payment, recurring membership billing will occur on the 5th of each month.
You are responsible for maintaining a valid payment method on file. Failure to do so may result in interruption of services, delayed prescriptions, cancellation of appointments, or termination from the program.
Membership Cancellation
Memberships require a minimum 30-day written cancellation notice. Cancellation requests must be submitted in writing through the website, patient portal, or email.
Your membership remains active and billing continues during the 30-day notice period. No partial or prorated refunds will be issued for unused services, time, or medications.
Failure to provide proper notice will result in continued billing until notice is received and processed.
PAYMENT DISPUTES AND CHARGEBACKS
By signing this agreement, you acknowledge that you are authorizing Pedrotel LLC DBA Thunder Performance Testosterone to charge your payment method for services, memberships, medications, and related healthcare expenses in accordance with this policy.
You agree not to initiate a payment dispute or chargeback without first contacting our office to attempt resolution. Most billing concerns can be resolved quickly.
Initiating a chargeback for a valid charge may result in:
• Immediate termination of services
• Suspension of prescription refills
• Referral of the balance to collections
• Responsibility for administrative, collection, or legal fees where permitted by law
